What is the Patient and Family Partnership Council? 

In order to better serve the members of the community and our patients, Covenant Homecare established the Patient and Family Partnership Council to improve service quality and safety. 

Through the council, members of Covenant HomeCare, community members, and patient family members can collaborate to create a Covenant HomeCare that reflects the needs of the community at large.

The goal of the Covenant Patient and Family Partnership Council is to learn from the experiences of those involved from the first stage of HomeCare to the last. Community members share their experiences of involvement with Covenant HomeCare. In turn, Covenant HomeCare team members take note of concerns and seek to clarify any issues – a win-win for everyone! 

Why Should I Join the Patient and Family Partnership Council? 

Anyone who has been involved in the Covenant HomeCare care process can apply to be a member of the council. The council explores many of the projects important to family members of patients, including revising admission information for patients and families, suggesting ways to improve home visits, or providing feedback for the website or marketing materials. 

Members of the Patient and Family Partnership Council 

Members of the Patient and Family Partnership Council exhibit a few common traits. If you enjoy the following, we think you might make a great fit. 

Characteristics: 

  • Enjoy working on a team.
  • Committed to ensuring quality care and patient safety.
  • Want to strengthen partnerships with our community and home health employees.
  • Can volunteer a few hours of your time monthly.

Once applications have been reviewed, selected candidates will complete an interview process. Those chosen for the council will receive information about orientation, meeting times and locations, and other details. Meetings will be at Covenant HomeCare and will typically be held monthly during lunchtime or evenings, with a meal provided.
